Radical hysterectomy with lymph node dissection represents the standard treatment for early-stage cervical cancer, alternative surgical approaches able to spare reproductive organs have been developed. In this context, vaginal radical trachelectomy (VRT) with laparoscopic lymphadenectomy represents the gold standard for fertility sparing surgery in early stage cervical cancer patients. In selected cases laparoscopic radical trachelectomy (LRT) may offer specific advantages. In this manuscript, we report a literature review and describe this surgical approach step by step.
Clinical Recommendation Radical Trachelectomy for Fertility Preservation in Patients With Early-Stage Cervical Cancer

Radical trachelectomy (RT) is a fertility-sparing procedure with the aim to provide adequate oncological safety to patients with cervical cancer while preserving their fertility. In the current review, indications, development of the procedure, technical aspects, preoperative and postoperative management, and oncological, fertility, and obstetric outcomes are discussed and studied with respect to whether the procedure is performed abdominally or vaginally. Complications of RT, staging, and more conservative alternatives to RT are discussed as well.
